[Az-Geocaching] State Land permit

Would someone happen to either have a PDF of the permit application or point me to where the link is to download it? I see the 'Group Recreational' permit on the list at http://www.land.state.az.us/programs/operations/applications.htm, but not the individual/family recreational use permit.
