Dirk,
I'm just echoing the information available on the SLD web site, where it
explicitly states,

"Only the Phoenix Office can issue the actual permit. Permit applications
obtained at the Flagstaff or Tucson Offices must be completed and mailed to
Phoenix for issuance",

highlighted in red at the bottom of this page:
