It's not easy designing something that has to withstand launch, endure space
for a few years, then survive the reentry and work flawlessly the first
time. There is only so much that can be done in simulators. Also the company
that makes the explosive charge for the chute is almost certainly not NASA
themselves. They could make 100s of charges that work fine and then one
fails. This is the real world. :)

Unbelievable..what a joke NASA is...if they were a private corporation,
they would have gone bankrupt a long time ago! What a waste of money, huh?
At least the crash pictures looked cool...
I guess they should have consulted Team Evilfish on how to make the chute
pop....
