The company I work for uses spamassassin. Work pretty well. I'm running spamassassin, and I wil never see any of their messages, althought I do feel sorry for anyone that isn't running it, especially if you e-mail address is at all old. I've been using my same address for 8 years, so the amount of spam I get is insane.
