You've got it. The generally accepted definition on the geocaching message boards is that Terrain deals with how difficult the cache is to physically reach, once you know where it is. Specialized equipment (4x4, scuba, rapelling, boat, etc.) is supposed to be a 5. Difficulty deals with how hard it is to figure out where to hunt (eg Shadow of the Pyramid, Picnic Leftovers, Water Ranch, etc.) or difficulty finding it once you are in the immediate area. Of course, burying is frowned on because of the number of holes that could be dug looking for the cache. Your first example would be a 5 / 1 while your second would be a 1 / 5, IMO.
